What is hair transplant and know your best hair transplant surgeon in Mumbai.

Hair Transplant is a surgical procedure which involves removal of hair follicle form the hair bearing sites (donor) to the bald (recipient) site. HT is currently the best treatment modality available to combat baldness.
Socially, professionally and personally an attractive physical countenance boosts ones’ self confidence and the same hampered by baldness has a significant psychological impact. The list of underlying cause of hairloss or baldness can be many. Few of which are unhealthy lifestyle, genetic propensity , stress, hormonal imbalance and medications.

The pattern of hair loss/ baldness is different for males and females aka Male pattern hair Loss (MPHL) and female pattern hair loss (FPHL) respectively. In males, the hairline is usually affected whereas in females, the crown portion undergoes hair thinning and ultimately leads to balding in that region. These patterns of hair loss requires immediate attention. Whether permanent or temporary, the developing baldness should be treated at the earliest.

Understanding the donor hair structure is equally important and crucial which will determine the final result of HT. Folliscopy done pre-operatively helps the surgeon to understand the density of the donor hair follicles which is a useful tool to explain the patient their condition and draw favourable expectations.

Individuals affected by baldness in their early 20s or mid 30s are good candidates for HT. This is because the vast availability of donor tissue improves the overall prognosis of HT. Moreover, the body accepts a grafted tissue at a faster rate in such individuals as compared to older patients. One more concerning fact with respect to age is the type of grafts extracted. The colour, thickness and number of the graft has a significant influence on the final HT result.

For younger patients reporting with developing baldness can undergo HT. This is to restore the bald spot with new hair and preventing complete baldness to develop. Now older individuals with comorbidities may have to take some extra care post surgery, as these individuals possess lesser threshold for the graft acceptance along with delayed healing. So the final result should be informed to the patient during examination.

The benefits of undergoing HT at an early stage wards an individual off the social dogma correlating baldness to ugliness. In such individuals, hair transplant needs to be planned in two stages, one at the early stage and the second at the later stages of life to successfully treat baldness.

2. Types of hair transplant

Hairloss is one of the major aesthetic issue concerning all age groups. Both temporary or permanent hairloss can accelerate to baldness in no time. Main aim is to restore hair growth and successfully beat baldness.

Available treatment modalities are broadly classified into two main categories: non-surgical and surgical intervention. The non-surgical approach being non-invasive delivers medicines in the form of shampoos, sprays or oral supplements. To note, only mild hairfall cases respond well to non-surgical approach.

Surgical intervention is a permanent approach to tackle baldness, aka Hair Transplantation (HT) or hair replacement or hair grafting. Here, hair follicles from the donor site is transplanted to the recipient site revamping the bald patch. Currently, two methods of HT are successfully executed by a cosmetic surgeon who specializes in HT.

After meticulous clinical examination and apace with patients’ expectations, either of the two surgical approaches i.e. Follicular Unit Extraction (FUE) and Follicular Unit Transplant (FUT) is described to the patient. Both FUE and FUT have remarkable results with their set forth downsides. Also, American Society of Plastic Surgeons reports high success rates with FUT and FUE techniques with overall satisfying rate of 8.3 on 10 over a period of 2.5 years.

The evolution of HT begins with ‘hair plugs’ where skin patches were transplanted from donor to recipient bald site. Later in the mid 1990’s, ‘FUT’ gained popularity wherein, hair grafts were harvested in a strip form mainly from the back of the head. FUT presented with 95% success rates but leaves a jarring scar at the donor site post operatively. Though the results post FUT technique are promising and natural looking, the scaring at the donor site remains to be the major drawback.
FUE presents with less post-operative patient morbidity as the technique utilises the micro-punching technique rather than linear stripping of hair follicles. As the blind punch of FUE is difficult to master, this technique was not greatly accepted in the early 2000s and was regarded to be inferior with regards to final outcomes as compared to FUT.

With the advent in technology, FUE for HT has gained popularity and has widely been in practice by surgeons for more than a decade. Currently, FUE is favoured over FUT for its low transection rate i.e. lesser damage to transplanted hair follicles during graft harvesting and less post-operative morbidity which works both in favour of the patient. Irrespective of the technique chosen, as a patient one should understand that the number of grafts required for best results and acquired during surgery majorly depends on the type of donor graft and also differs from patient to patient.

3. TREATMENT FOR OPEN PORES

Facial skin is very sensitive and majorly targets its inner constituent i.e collagen. Collagen promotes healing and tightens the skin. Pores present on the skin surface allows the skin cells to produce oil (sebum), sweat and contain hair follicles. These are naturally present on the skin surface and may appear small or large as per the skin type.

Direct relationship exists between pores and collagen i.e. active collagen activity prevents the facial pores to enlarge, also, with age as the collagen activity seizes, this reflects as enlarged pores on the face. Appearance of open pores on face is largely contributed to the shape it attains because of varies reasons that can be oily or acne prone skin (T-zone area), dry skin, over-exposure to sun and deficit collagen production.

Briefly, three types of open pores are noticed on different areas of face viz. :
O- shaped: usually in acne prone or oily skin,
U-shaped:  in areas of dry skin and
Y- shaped: on the cheeks with less collagen content.
Open pores are basically scars which can be managed. Baseline of any treatment modality is to promote collagen production that promotes healing of the formed scar i.e. open pores on the face.

When you report to the doctor with open pores, one needs to know that pores cannot be closed permanently, however, its appearance can be worked on. Your skin type and the underlying cause of open pores is mainly addressed when planning any approach to manage open pores. A wide range of treatment options are available which ranges from topical drug application to surgical intervention.

On a preventive note, sunscreen application, self hydration and removal of makeup  if followed religiously does not clog the pores and maintains it well.

Topical application of retinoids, AHAs, Vitamin C, niacinamide, cleansers with salicylic acid followed by daily exfoliation and clay mask are non-surgical options to treat mild open pores.

Coming to more invasive techniques i.e. derma rollers, laser treatment, TCA Cross  (chemical reconstruction of skin scars using trichloroacetic acid) and AFT technology (photofacial treatment) are opted for patients with larger pores. These procedures effectively reduce the size of the pores and enhance collagen remodelling.  We at Clinique Internationale use , Bison More-Xel co2 fractional laser for skin resurfacing and thus pores closure . This laser works by remodelling of collagen and thus tightening the skin .

Role of Co2 fractional lasers for open pores:
Fractional CO2 is FDA approved treatment for treating open pores and acne scars which enhances skin texture and delivers even skin tone by facial resurfacing. SPECTRA, the fractional CO2 introduced by BIOSON
Acne scars exposed to CO2 laser causes thermal damage beneath the scarring tissue promotes remodelling of collagen thus reducing acne scaring. The superior remodelling capacity of CO2 laser as compared to derma abrasion and chemical peeling results in effective resurfacing of skin by continues collagen production. The overall output of fractional CO2 for acne scars delivers skin tightening and a rejunivated look.

Dr. Bhavesh Gupta at Clinique Internationale, Kharghar self-reliantly practices laser  for skin resurfacing and open pores. All in all, after thorough clinical examination Dr. Bhavesh gupta and team formerly promote proper nutrition and skin hydration regime to understand patient’s skin type and needs and suggests a proper treatment for open pores.

4. CARBON FACIAL

Skin undergoes cellular changes when exposed to sun. The signs of ageing i.e. wrinkles, fine lines, hyperpigmentation, open pores are visible at an early stage as a result of sun damage. Carbon laser peel also known as Hollywood peel provides instant glow and targets open pores, acne and improves overall skin texture.

CARBON LASER PEEL is an effective rejuvenating skin treatment for instant glow well-aimed to reduce pigmentation, acne scars, clogged pores, removes dead skin and leaves one with radiant and glowing skin. The laser energy used in carbon laser peel helps remove the harsh dirt layer on the skin, exposes the underlying fresh skin surface and competently delivers even skin tone.

In simple words, Carbon facial peel is a non-invasive, ready to go treatment available to enhance your look within 30-35 minutes. Carbon facial peel also known as Carbon laser facial uses laser light that acts cellularly and, effectively delivers a peeling effect. In this way, carbon facial peel proves to be an effective treatment for congested skin.
Also, the laser light of 1064nm tacitly and swiftly reduces inflammation induced by sun damage thus soothing the skin cells to help it look younger.
Steps of Carbon laser peel At CLINIQUE INTERNATIONALE :
Firstly, the skin is cleansed to remove oil, dirt or makeup.
A thin layer of liquid carbon is equally applied on the face. Eyes are shielded with a protective eyewear.
Laser light of 1064nm is used to peel the carbon mask off painlessly.
Once all the carbon particles are completely taken off by the laser light, face is cleansed and prepared for clay mask,
Following this, a black clay alginate mask is applied all over the face.
This mask soothes and tightens the skin delivering a younger look. The result of carbon laser peel lasts for about 2-3 weeks only if the open pores and acnes are properly protected against sun light. So a proper aftercare regime has to be followed throughly.
After the procedure we recommend to use DEPIWHITE peel-off mask thrice a week as a home-care regime.
Post peel precautions to be followed:
Avoid sunlight for at least 48 hours post carbon laser peel.
Apply sunscreen of atleast SPF 35 to protect the freshly exposed skin.
Avoid using facial scrubs.
Do not apply any chemical peel.
Avoid use of any topical antibiotics for acne.

5. WART AND WART REMOVAL

People encounter abnormal skin growth in various part of the body but those on the face becomes displeasing for many. These non-cancerous skin growth are commonly known as warts. Warts usually are painless but those under the feet can cause physical discomfort.

Skin cells which are infected by human papilloma virus (HPV) multiply more than normal. Warts usually grows as a non-defined mass on the skin surface. The upper layer of the skin i.e. epidermis is usually affected by the virus. They tend to have a rough surface and seldom manifests with dark spots signifying its high vascular supply. With 150 variants of HPV, 10 of which are responsible for causing cutaneous warts.

Warts can be contagious and mainly spreads through breaks in the skin. Unlike bacterial infections, the virus causing warts doesn’t eradicate completely. The virus remains in the upper layer of the skin and spreads  spontaneous from one part to the other. So its highly recommended to wash your hands once you have come in contact with it.

The types of warts are

Common warts: raised with rough surface commonly found under fingernail or toenails which are difficult to treat.
Plantar warts: also known as mosaic warts are often found on the soles of the feet, may appear flat with added discomfort while walking
Flat warts: mostly seen on the face, appear small in size and more in number.

Flat warts cause aesthetic concern as they appear in clusters mainly around the eyes and seldom on the hands. The non-invasive option ’wait and watch’ is often used to evaluate the activity of the wart. The prevailing virus within these warts tend to invade the surrounding healthy tissues. So its advisable to treat warts surgically and remove the offending virus in the epidermal layer.

Treatments available at CLINIQUE INTERNATIONALE for wart removal largely depends on the overall health of the patient and the type of wart. Doctor can suggest anyone of the following options following thorough clinical examination:

We at Clinique internationale , use More-xel (A US-FDA) approved Co2 laser for wart removal which makes the procedure painless and comfortable for all our patients .

6. FACIAL RESURFACING BY FRACTIONAL LASER

Facial skin being very sensitive undergoes paramount changes due to ageing, sun-damage and acne. The damage associated with these appear as wrinkles, rhytids, uneven skin tone and texture, large open pores hyperpigmentation and scarring. Treating these conditions becomes challenging as there hasn’t been any one procedure targeting all in one session.

Lately, the non-ablative and ablative lasers have proven effective for treating scars and ageing signs. Non-ablative laser carry the advantage of having less post-procedural discomfort but has been incapacitated over ablative lasers i.e. CO2 lasers. CO2 laser since 1990s have been implicated for cutting and tissue destruction with the depth penetration of 400-500 μm.

In 2004, ablative CO2 lasers was introduced and since then it has been a boon to cosmetic surgeons. The ablative nature of CO2 laser coagulates the blood vessels thus minimising bleeding during the procedure. Thermal injury to the scarred tissue promotes contraction of the collagen fibres and promotes collagen remodelling. The overall effect of collagen contraction presents as skin tightening giving a rejuvenated look.

The energy emitted by CO2 laser is absorbed by intracellular water which ablates the most superficial layer of the epidermis subsequently resulting in re-epithelialization  in the dermis. As the heat produced by CO2 laser is more, there is greater coagulation and this photocoagulation stimulated deeper layer of the dermis and produces a more even and sterile surface.
Ageing skin represents with more laxity, wrinkles, uneven skin tone and texture moreover, photoageing of the forehead, perioral and periorbital areas have shown improvement with CO2 laser by stimulating collagen remodelling in these areas. Treatment of Acne scars with CO2 laser causes thermal damage beneath the scarring tissue resulting in remodelling of collagen thus reducing acne scaring. The superior remodelling capacity of CO2 laser as compared to derma abrasion and chemical peeling results in effective resurfacing of skin by continues collagen production.

Active acne, active infections like herpes, burns and history of oral isotretinoin, vitiligo, psoriasis and any autoimmune diseases are few conditions where laser CO2 resurfacing is contraindicated. The cause of active acne which can be either hormonal or stress induced should be addressed  prior to planning of laser CO2 resurfacing.

Local topical anesthesia is used for facial CO2 laser resurfacing. For smaller areas or fractional resurfacing, a series of topical anesthetic applications followed by a local anesthetic block or infiltration may be sufficient for patient comfort. Eyewear, or laser goggles may be chosen for patient eye safety. Laser light is then exposed to the face which targets deeper scars and activates collagen remodelling.

Dr. Bhavesh Gupta, currently is the first cosmetic surgeon in Navi Mumbai to install and use MORE-XEL CO2 Fractional laser by Bison, South Korea.
